What is the USPHS Dog Tags format?
Our replica USPHS Dog Tags have Embossed (raised) text on a 2" x 1⅛" dull stainless steel Dog Tag, rolled edge up, hole on left with the suggested regulation format:

- [First Name] [Initial]. (if last name is too long on line 1)
- [DoD ID Number] (or SS number)
- USPHS [Blood Group] [Rh Factor]
- [Religion] (or NO PREFERENCE)
The template is almost the same format as modern Army Dog Tags, with the difference being that the letters USPHS are inserted before the blood type.
